- Most Recent Physical/TB
- Information regarding the last dental visit
- Copy ID
- Copy of Social Security card
- Copy of Birth Certificate
- Copy of Insurance Card
- List of Medication (physician orders/scripts/medication list from doctor)
- Legal Status (Guardianship Paperwork)
- SIS
- CSB parts 1-3
- VIDES
- Virginia Informed Choice
- Any Medical and/or Behavioral Protocols (if applicable)
- Behavioral Plan (if applicable)
- List of treating Physicians with contact information
